Here is what you need to do:
Provide bleaching trays and gel (along with a screening examination to determine if they are good candidates for bleaching services) for patients who make a $250 donation to the Gwinnett Medical Center Foundation. The patient will pay your “kit fee” to cover the costs of materials. Absolutely no profit is to be made from the kit fee. We discourage the participating dentists from soliciting patients through the effort and ask that you discourage the participating patients from changing from their current family dentist. This effort is for charitable purposes only.

For the past three years, proceeds have been donated to the Gwinnett Medical Center Foundation to help fund projects in our own community:

- How are funds raised?
Funds are raised by exchanging professional home bleaching services for a $250 donation to the GMC Foundation (a minimal kit fee is also charged to cover bleaching kits).
- What is the chief goal of “Brighter Smiles for Brighter Futures”?
Our goal this year is to recruit 100 dentists with a combined monetary goal of $200,000. Monies raised will go toward the purchase of a much needed breast ultrasound machine and items to improve the comfort of woman who are being treated at the mammography center - How will we get patients to participate in the campaign?
Most of the patients that will participate will come from our current patient base with additional patients referred from news articles. The dental staff is vital to making this effort a success in your practice. - Will there be any advertising done?
Articles about the effort have appeared in the Gwinnett Daily Post, Gwinnett Magazine, Atlanta Journal, medical magazines and Gwinnett Medical Center’s publications and Web site (www.brightersmiles.org). - Do the patients need to be patients of record?
No – we have the patient sign an abbreviated medical history and release form. - How long does the campaign run?
The campaign starts October 1, 2008 - January 31, 2009 - What is the monetary goal to be raised by each office?
A goal of $2500 (10 patients) is recommended, but each office is free to do more or less. This goal is easily attainable with good staff participation. This goal should not be a deterrent for participation. - How much do we charge for the donation?
$125 per arch or $250 for both upper and lower arches, plus an additional charge to cover the cost of the bleaching kit. The office should make no profit from the bleaching. - How do we collect the money and get it to GMC Foundation staff?
See the enclosed information sheet. - How much of the donation goes to the Gwinnett Medical Center Foundation?
100% - Is there a tax deduction for the patient’s contribution?
Yes, the patient’s check is their receipt. The Gwinnett Medical Center Foundation 501c 3 tax ID # is 58-1828486. Consult your tax professional for confirmation. - Is there a Web-site for more information?
